Emergency Medical Technician Program

The Application Process has 5 steps. Please make sure you read carefully and complete all parts of the application.

Students wanting to apply for the EMT program must complete the following steps:

1. Complete the CTE Center Interest Form. We use this to count the number of interested students and to have a way of contacting you. 
2. Complete the Dual Credit EMT Application Form
   • *Important Note: The Application Form is owned by Collin College. You will need to use a Google Account that is NOT your Plano ISD Google Account to complete it. If you cannot access the form, double check which account you are using. 
   • The main portion of the dual credit EMT application process is the Student Application Form. Each student will complete the following sections within the form:
     • Demographic Information: Please ensure you provide ACCURATE demographic and contact information in this section of the form. The information you provide here is how you will be contacted with the results of this qualification process. We are not responsible for typos in email addresses.
     • Functional Abilities/Special Circumstances: Please review the functional abilities section of the application to determine if you meet the required criteria. If you have questions as to whether or not you meet the functional abilities requirements, please contact our office at EMSinfo@Collin.edu.
     • Short Answer Questions: Students must complete two short-answer questions as part of their application. All responses must be written entirely by you - AI-generated answers are not allowed. It’s strongly recommended that you type and save your answers in another document first so you can check spelling, grammar, and punctuation. Keep your answers clear, original, and to the point, using at least two complete sentences but no more than 100 words.
     • Current Grades from Online Grade Book and Unofficial Transcript: Please provide a screenshot of your current grades from your online grade book or student portal. Be sure the image includes your name and all current classes and grades. Please also provide a copy of your current unofficial high school transcript, available from your high school counselor. Please save these documents as PDF files with the following titles. FirstName_LastName_Grades.pdf and FirstName_LastName_Transcript.pdf (Example: John_Doe_Grades.pdf)
   • Dual Credit EMT Application Form
3. Apply to Collin College unless you are already a dual credit student.
4. Teacher Recommendations
   • Each student is required to have two (2) total teacher recommendations. 
     • One recommendation from a current Health Sciences or core subject teacher (math, english, science, or history/social studies)
     • One recommendation from a current core subject teacher
   • Please provide your teacher a link to the EMT Teacher Recommendation Form AND the Teacher Verification Form (also available on the handout from the EMT Program Information Session)
   • The form is to be completed by teachers ONLY; this form does require the teacher to submit their school email address. The responses made by your teachers are confidential and are not shared with students.
   • Please DO NOT attempt to complete and/or submit this form by yourself, or you will be
     disqualified from the clinical qualification process.
   • Please ensure your teacher submits their answers by the application deadline. Teacher recommendations will not be accepted after the application deadline date.
   • Students are responsible for following up with their teachers to ensure completion. We will not email your teacher directly to request that they complete the form. We will not respond to email requests regarding teacher recommendation submissions or emails related to teacher
     recommendation responses.
5. Collin College Dual Credit Permission Form
   • Make sure all three sections of the form are completed, then turn in to your counselor. 
   • Each section needs to be completed by a specific person. Double check before you complete and submit.

Don't forget to hit submit on your application form! All applications close on February 6th, 2026 at 5pm (CST)

What do I do after I apply?

Following the submission deadline, all student information will be reviewed by a committee of faculty and staff from the EMS Department at Collin College. Students who complete the application correctly, submit all required attachments, and have two teacher recommendations will be scheduled for an interview. These interviews may be held virtually, and you will be assigned a date and time to attend.

Be sure to check the email address you listed on your application, including your junk or spam folder, regularly. All updates about your EMT program acceptance, next steps, and important deadlines will be sent to that email. Some messages require a quick response, so don’t miss them. If you do not reply to your program offer by the stated deadline, your seat may be given to a student on the waitlist.

Staying on top of your application email is the only way to secure your spot in the program.
